In accordance with State and Federal Law, UCSC provides reasonable accommodations for qualified students with disabilities in order to ensure equal access to all services, programs, and activities of the University.

Student responsibilities:

  1. Students contact the DRC to determine their eligibility for accommodations. When approved by DRC, they will receive their Accommodation Authorization form
  2. Students then notify their instructor during office hours or after class of their accommodations, and provide their instructor with their Accommodation Authorization form.
  3. Please note that it is the student's responsibility to contact the instructor about their accommodations. If they do not contact their instructor, accommodations will not be made.
  4. Students should submit their requests to faculty no later than 7days before a regular exam and 14 days            before a final exam.

Instructor responsibilities are as follows:  

1.  Instructor makes an announcement during the first two weeks of class, asking students to come to them after class or during office hours to discuss testing accommodations. Additionally, the instructor should make an announcement a week before each exam, for any newly approved DRC students. It is also recommended that instructors post DRC information on their syllabi.  For example: 

UC Santa Cruz is committed to creating an academic environment that supports its diverse student body. If you are a student with a disability who requires accommodations to achieve equal access in this course, please submit your Accommodation Authorization Letter from the Disability Resource Center (DRC) to me privately during my office hours or by appointment, preferably within the first two weeks of the quarter. At this time, I would also like us to discuss ways we can ensure your full participation in the course. 2.  Upon receipt of the student's Accommodation Authorization form, the instructor delivers the form to Instructional Support Services (ISS)- E2-298.

Note: Instructors should submit  accommodation requests to ISS no later than 7 days before an exam and 14 days before final. Last minute requests may get rejected for lack of room or for lack of enough proctors to administer exams.

3.  Instructor submits all testing dates as soon as known via Google Web Form found here.

4.  Instructor delivers HARD copies of  tests for each student to Instructional Support in E2-298 24 hours prior to the exam time.

5.  Exam accommodations are provided to ensure equal academic access for qualified students with disabilities. These students are authorized for test accommodations in accordance with both Federal regulations and University policy. 

    Arrangements to be made by the instructor include: 

a) Be available (or designate a TA) for questions during exams, if typically provided to the rest of the class.

b) The DRC can provide test scribes or readers, as authorized for students with disability-related needs with advanced notice from you (at least one week prior to exams if possible). Contact DRC: Email: drc@ucsc.edu; Phone: (831) 459-2089


*Note: Test proctors are not required by law.  It is up to the instructor whether or not to designate one.

    • When an Accommodation Authorization form is delivered by the instructor to ISS, ISS will discuss with the instructor whether they will require a test proctor and the testing dates and times. 
    • If a test proctor IS NOT required, ISS will schedule a room and a time (information received from instructor) and email student and cc the instructor with room information.  Instructor or their designate will meet the student(s) in the scheduled exam room and provide exam.  
    • If instructor IS requiring a test proctor, ISS will schedule a room, time and test proctor and google calendar invitation to student and  the instructor with testing information.  The test proctor will meet the student(s) in the scheduled exam room.
